Say the difficult thing in week two
Every overrun programme contains a moment, early, where somebody knew. The cost of raising it then and the cost of raising it at month six differ by an order of magnitude.

The information exists early
Scope problems are usually visible in the first fortnight. What is missing is not the information but the willingness to spend relationship capital on it before any has been earned.
It is cheaper for both sides
A client who hears a scope problem in week two can still act on it. The same client at month six is choosing between sunk cost and admission. Raising it early is not bravery, it is arithmetic.
